Is it necessary to fill up any Form for availing insurance coverage at the time of account opening?

It is not necessary to fill up any form by the customer for Accidental insurance. Medical insurance

claimant customer needs to fill the form at the time of claim only. However, for Critical illness coverage a separate form need to be filled at the time of account opening.
